In this care context, punctuated by urgency and vital risk, I work on psychotraumatic risks, vulnerabilities and psychological resources. My work is based on the development of prevention tools and psychological care systems.  The results of my work the practice of psychology in hospitals, to improve the care offered and support for health care professionals.

1/DPSYREA (2024-2029) : Psychological interventions in intensive care: Roles and clinical practices of the psychologist

 Summary : Intensive care (ICU) has a major impact on the psychological balance of patients, relatives and caregivers. The pandemic has accentuated these vulnerabilities. Although decree 2022 makes the presence of psychologists in ICU mandatory, no scientific studies or professional recommendations are available on the practice of psychologists in ICU in France. There is therefore an urgent need to develop psychological services in ICU. DPSY-REA aims to characterize and evaluate psychological intervention in ICU, and to disseminate them in the clinical practice and training.

2/ PRODON Quali study (2022-2024): Collaborative approach to the analysis of relatives’ refusal of organ donation in the context of the Maastricht III category of potential donors: a randomized, controlled, and open study.

 Summary

The “Maastricht III” controlled organ donation procedure (DDACM3) has been promoted in many countries for more than 10 years. This procedure requires collaboration between ICU teams and the hospital teams coordinating organ and tissue procurement. Although it is intended to facilitate donor identification and reduce the percentage of refusals, little is known about how these teams work together and how family members experience this procedure. This qualitative study aims to analyze the experiences of family members and healthcare professionals with the DDACM3 in order to improve practices and strengthen the training of coordination teams and ICU physicians involved in these activities.

3/ COPREA study (2022-2025): Evaluation of a post-ICU interdisciplinary consultation device

Summary

The aim of this prospective, multi-center, longitudinal, randomized, interventional study is to establish a post-ICU interdisciplinary consultation (IPRC) and to evaluate its impact on patients in terms of psychotraumatic disorders, anxiety-depressive disorders and quality of life. The study also seeks to assess the impact of post-ICU consultations on ICU healthcare practices and how the teams feel about their work by designing and administering a satisfaction questionnaire about these consultations.

The aim of this study is to improve the post-ICU care of patients through better identification of disorders, more appropriate referrals and stronger links with outpatient medicine. COPREA aims to consolidate the role of psychological support during and after a patient’s hospital stay.

4/ NOSTRESS study (2022-2025): Monitoring and assessment of stressors in new intensive care patients. Study in France and Italy

Summary

Arriving in an ICU as a new intern, nurse or orderly means having to deal with a specific care context with specific requirements in terms of technical knowledge, responsiveness to emergency situations and relationships between healthcare professionals, patients, and families. Through this study, we aim to assess the relationship between work-related stress factors and quality of life among new entrants, with a view to developing a support and training program for new ICU interns and nurses upon their arrival in the department. Therefore, our main objective is to identify the stressors in the ICU for new arrivals (interns and nurses) using the PS-ICU questionnaire (Laurent, Fournier, Lheureux, Martin Delgado, et al., 2021) and to conduct a longitudinal study on the impact of these "stressors" on the work-related quality of life (WRQoL).

 5/ PsyCOVID study (2020-2023): Psychological impact of the COVID crisis on ICU healthcare professionals

 Summary

The ICU plays a special role in our healthcare system. The urgency of clinical situations, the proportion of deaths encountered, and the daily workload are all likely to cause suffering. The public health crisis associated with SARS-COV-2 was unprecedented and led to an unprecedented mobilization of professionals, particularly in the ICU. In this environment, the mental health of healthcare professionals has become more of a concern than ever before, as highlighted by the CCNE. The main aim of this multi-center, observational, longitudinal study is to identify and understand the psychological impact of the COVID-19 crisis and the psychological resources of ICU medical and paramedical professionals during and after the crisis, using a mixed methods approach.

6/ RESIREA study (2020-2023): Psychological trauma and resilience processes in patients after an ICU stay for severe acute pathologies requiring mechanical ventilation.

Summary

While the psychotraumatic effects of an ICU stay have been widely described in the literature, few studies have examined the ability of patients to cope with this upheaval and to regain psychological equilibrium after an ICU stay. In this study, we aim to assess patients’ resilience after an ICU stay. We will also examine the role of perceived social support and how patients’ perceptions of their illness affect resilience.

7/ ENVIFAR study (2018-2022): The child visitor in intensive care: evaluation, from a psychological perspective, of the impact on children of visiting a parent hospitalized in intensive care.

Summary

The presence of relatives in the ICU is no longer controversial. This is based on an extensive and recent literature highlighting the benefits of this presence for both patients and their families (Laurent, Leclerc, et al., 2019). Today, however, the issue of allowing children and adolescents to visit the ICU remains sensitive for both parents and healthcare staff, leading many units to prohibit visits by young children. This is justified by the desire of parents and healthcare professionals to protect the child from an inappropriate and shocking environment.

The aims of this multi-center, prospective, mixed methods study are: 1/ To gain an understanding of the experience of visiting an ICU and its psychotraumatic effects on the child. 2/ To identify and understand the role of parental, family and caregiver support during the child’s visit. 3/ To identify the needs and expectations of the child, parents, and healthcare professionals involved in the visit.

8/ PS-ICU study (2016-2021): Construction and validation of an ICU-specific stress scale: Perceived stress in intensive care units

 PS-ICU 2 (2022-2024): Reduction of PS-ICU for routine use (Coordination with Florent Lheureux, Alicia Fournier, Gilles Capellier) 

Summary 

ICUs are an ideal setting for the emergence of occupational stressors. Although a number of tools are currently used to assess stress in ICU staff, these tools do not take into account the specific nature of the work (Laurent, Lheureux, et al., 2020). The aim of this international, multi-center study was to develop a scale to measure the perceived stress of ICU professionals.

Interviews were conducted with 166 caregivers (84 nurses and 81 physicians) in four countries (France, Italy, Spain, Canada). These interviews were recorded, transcribed, and thematically analyzed to identify stressors. A first version of the scale was pre-tested on 70 professionals (30 physicians and 40 nurses) in four countries (France, Italy, Spain, Canada). We then conducted qualitative and quantitative analyses to select the most relevant items.

We identified 99 ICU-specific stressors, grouped into eight main themes: (1) patient-related stress, (2) task-related stress, (3) institutional stress, (4) team-related stress, (5) department-related stress, (6) personal stress, (7) stress related to the patient’s family, and (8) work-related stress. After pretesting, 50 items were selected to construct the PS-ICU scale.

The objective of PS-ICU 2 is to propose a short version of the PS-ICU scale, validated for routine use in ICUs.

9/ MeCOPES (2020-2023) Adaptation mechanisms activated during pandemic-induced individual economic shock

Summary

The pandemic caused real economic, social and psychological shocks for some people. Many people were forced to cope with job insecurity, unemployment, loss of income or housing, or even bankruptcy. These ordeals had a significant impact on their overall mental health and well-being.

The aim of the MeCOPES study is to identify the factors that can act as a protective mechanism for individuals in times of crisis, be it economic, social or psychological. Our multiple European partners and our multilingual questionnaire will allow us to compare data across Europe and identify protective factors that transcend national contexts and cultural differences.

10/ Study: Psychosocial factors predicting relapse in Crohn’s disease (2012-2016)

 Summary

The aim of this multi-center, prospective, longitudinal study was to identify psychosocial factors predictive of relapse using a clinical approach.  The second objective was to understand the psychological issues involved in the experience of Crohn’s disease.  Our quantitative and qualitative approach allowed us to collect data on psychosocial factors through questionnaires administered to 145 adult subjects in remission from Crohn’s disease. At the same time, we conducted 33 interviews, which were then thematically analyzed.

Summary

The main objective of this multi-center, observational, qualitative study was to identify the psychological reactions of ICU professionals following a medical error and to understand their evolution. To identify the psychological defense mechanisms used by professionals, both individually and collectively, to cope with errors.

Licence 3 : Méthodologie et épistémologie de la recherche qualitative en psychologie  :  Cet enseignement porte sur les méthodes qualitatives en recherche en psychologie. Nous abordons l’épistémologie des méthodes en psychologie clinique et leurs objets d'investigation, les productions de la subjectivité individuelle. La méthodologie qualitative est ici travaillée avec l’étude de la méthode inductive et hypothético-déductive. Cet enseignement a pour objectif de familiariser les étudiants avec les outils de recherche qualitative ; d’acquérir une démarche de recherche ; et de donner les bases concernant l’analyse des données qualitatives. Les dimensions éthiques sont également travaillées tout au long du processus de recherche.

Licence 3 : Clinique du groupe :  L’objectif de cet enseignement est de former les étudiants à la clinique du groupe. Différents concepts qui émanent de l’approche psychanalytique groupale seront étudiés et discutés (groupalité psychique, identification, narcissisme groupal, appareil psychique groupal, mentalité de groupe…). Il est enfin question des applications possibles de cette approche clinique groupale au sein des dispositifs thérapeutiques.

Master 1 : Corps et psyché :  Cet enseignement, en deux volets « corps et psyché » master 1 et « dispositifs de soins à l’hôpital » en master 2, a pour objectif de poser les bases théoriques sur les enjeux psychiques de la maladie grave sur le corps, l’image du corps, les enveloppes psychiques. Nous étudions également l’impact traumatique de la maladie, le remaniement et rupture identitaire et du travail de la maladie.

Master 1 : Traumatisme psychique, crise et changement. Cet enseignement aborde les bases historiques, étiologiques et nosographiques du psychotraumatisme en lien avec les travaux de la psychiatrie de guerre et la psychodynamique. Le traumatisme est ensuite travaillé selon ses spécificités au regard de la maladie grave en lien l’excès de douleur psychique, l’effraction psychique et corporelle. La question de la résilience est également étudiée en lien avec les concepts de mentalisation, symbolisation, stratégies défensives et croissance post-traumatique.

Master 2 : Dispositifs de soin à l’hôpital  : L’impact de la maladie grave est appréhendé selon la triangulation : patient, famille et soignants. L’objectif est de former les futurs psychologues à la pratique de soins psychologiques dans les services de soins somatiques. De mesurer les enjeux des dispositifs de soins psychiques auprès des patients, proches et soignants et de comprendre la place du psychologue dans ces dispositifs. La clinique des interstices, le transfert diffracté, les thérapies bi et multifocales seront des points d’ancrage pour penser des dispositifs adaptés au sein des services de soins somatiques.
